This article covers how you can enter your full name history into the Verifile Background Screening form.

- Access to the Verifile Background Screening form

 

GUIDANCE:

When completing the online form, one of the first steps will be to provide your full name history. This section only requires you to provide us with legal names—you do not need to provide any names you did not legally change to.

To begin with, you will need to provide your full current name.



If you previously completed the Digital ID section (where you were asked to scan your ID documents) this will likely already be pre-populated for you.

Once you have added your current name, you will need to enter the date you began using this name, or alternatively tick 'from birth'.

Next, you will need to select whether you have been known by any previous names. 



By clicking yes, you will now have the option to add any previous names.



Similarly to how you added your current name, you will now need to add the full previous name, along with the date you started using this name, and the date you stopped using the same name.

To add any further previous names, you will need to click '+Add additional names'.



Please ensure that you have provided all of your previous names, from birth to current. The form will not allow any gaps between these names, so you will need to be careful to ensure that the dates are in a timeline order.

We recommend adding the previous names in order, to help avoid any mistakes.
