The online Background Screening Form is your portal for providing us with all the necessary information we request for your background check.
You will need to fill out the form with your personal information, as well as any information that is relevant for the checks your employer has asked us to carry out.
This form is the starting point for our background screening process.
When filling out the online form, you do not need to fill out every section if you do not hold the relevant information.
Completing any field that is marked with a red asterisk or drop-down is mandatory.
You are not required to complete any segments not marked by an asterisk.
However, you should still strive to provide as much information as you can, including anything that is not mandatory.
This will help ensure that your checks run smoothly and quickly.
When trying to progress to the next page, it will mark any mandatory sections that you have not filled out with an error report at the top of your page.
You cannot proceed or submit the form until you have completed these sections.
While basic information, such as name and date of birth, will be mandatory for every candidate, some checks will have additional mandatory information.
Please ensure you keep an eye out for the red asterisks on all pages of your Online Background Screening Form.
